I would ask the leadership team some provocative questions like:

  • Is this a sign that people don’t want to expose problems?
  • How does leadership typically react when there is a red or yellow dot on the board?
  • How might leader behavior be contributing to the fact that everything is “green”?
  • What can leadership do to make it to make OK to make problems visible?

Hopefully, these leaders are motivated to change the behavior that may be contributing to this “cultural norm”.
