My main experience is applying FMEA to Six Sigma Projects in both the Analyze Phase – to assess current risk, and in the Improve Phase – to assess the risk of the improvements. In particular it’s key to conduct an FMEA prior to a Pilot and/or prior to Implementation. I would consider it a standard Six Sigma tool mainly associated with vetting the solutions in the Improve Phase.
